Report to Congress on Abnormal Occurrences
To find the annual Report to Congress on Abnormal Occurrences, pick a year and then conduct a search for abnormal within the document titles. An abnormal occurrence is defined as "an unscheduled incident or event that the U.S. Nuclear Regulatory Commission (NRC) determines to be significant from the standpoint of public health or safety." Reports cover imaging and radiation accidents, errors, and overexposures.
